Go to the ER right away if the sports injury involves:

Head, neck, or spine concerns

  • Knocked out (even briefly), “not acting right,” confusion, or unusual behavior

  • Severe or worsening headache

  • Repeated vomiting

  • Seizure, fainting, trouble walking, weakness/numbness, or slurred speech

  • Neck pain after a collision or fall

Breathing or chest concerns

  • Trouble breathing, wheezing, or bluish lips/face

  • Chest pain after impact, chest tightness, or coughing up blood

Possible broken bone, dislocation, or circulation problem

  • Obvious deformity, bone showing through the skin, or the joint looks “out of place”

  • Can’t bear weight or can’t use the arm/leg normally

  • Severe swelling, intense pain, or rapidly increasing bruising

  • Numbness/tingling, a cold hand/foot, or the limb looks pale/blue

Bleeding or wound concerns

  • Bleeding that won’t stop with firm pressure

  • Deep cuts, gaping wounds, or a dirty puncture wound (especially if tetanus status is uncertain)

Heat illness during sports

  • Confusion, collapse/fainting, seizure, or extreme weakness during/after activity in the heat

  • Hot skin with altered behavior or severe symptoms

Why sports injuries can be serious

Sports injuries aren’t only sprains. Depending on the impact and symptoms, we may be concerned about fractures, dislocations, ligament tears, head injury/concussion, internal injury, or heat-related illness. The safest rule is simple: if function is limited (can’t use it), the pain is severe, or symptoms are escalating, get evaluated.


ER or urgent care?

Choose the ER when there are red-flags (head injury symptoms, severe pain, deformity, inability to bear weight, uncontrolled bleeding, breathing/chest symptoms, fainting, or heat illness).
Consider urgent care for mild sprains/strains and minor injuries when:

  • Pain is mild to moderate and improving

  • They can walk/use the limb

  • There’s no deformity, numbness, severe swelling, or head injury symptoms

If you’re unsure, it’s reasonable to choose ER care—especially for children.
