Go to the ER right away if cough or sore throat comes with:
Trouble breathing or shortness of breath
Trouble swallowing, drooling, or you cannot swallow fluids

A muffled “hot potato” voice, severe one-sided throat swelling, or trouble opening the mouth (can suggest a deep throat infection/abscess)
High fever with worsening symptoms or you look/feel seriously ill (especially if you’re dehydrated)
A high-pitched noise when breathing in (stridor) or rapidly worsening symptoms

I was greeted and made aware that I might have to pay self pay if the Dr deemed it not an emergency. After getting an ekg the Dr said it wasn’t an emergency and they proposed I pay over $1800 to be examined. The Dr said what do you have Medicaid, making an unfair assumption. I replied no I have Aetna insurance. I politely declined and had to go somewhere else. Most kids improve with rest, fluids, and pediatric follow-up — but airway and breathing red flags are never “wait and see.”
Bring your child to the ER now if they have:
Trouble breathing, fast/working-hard breathing, or stridor
Trouble swallowing, drooling, or inability to take fluids
Blood in saliva/phlegm or coughing up blood
Fever in an infant under 3 months (100.4°F / 38°C or higher) — this age group should be seen urgently
Extreme sleepiness, dehydration, or a child who looks significantly unwell
Why cough and sore throat can become serious
A sore throat can come from common viruses — but it can also be caused by bacterial infections (like strep), deep infections (like an abscess), or dangerous swelling (like epiglottitis). Some of these can block the airway, which is a true emergency.
A cough can also be “just a cold,” but when paired with breathing difficulty, blood, or severe systemic symptoms, it deserves urgent evaluation.
Higher-risk patients: don’t “tough it out”
If your immune system is weakened, infections can become severe faster. Seek urgent care (often ER-level) for cough/sore throat plus red-flag symptoms if you have:
Cancer treatment or significant immune suppression
HIV/AIDS
Recent transplant or anti-rejection medications
A severe chronic illness or recent serious infection
(Clinical reality: immunocompromised patients have a lower margin for safe home observation. If they have breathing/swallowing issues or systemic symptoms, ER evaluation is appropriate.)
ER or urgent care?
Choose the ER if any of the red flags above are present: breathing trouble, swallowing difficulty/drooling, blood, stridor, rapidly worsening symptoms, or very young infant fever.
Consider urgent care/primary care if symptoms are mild, you’re breathing comfortably, swallowing normally, and you don’t feel seriously ill — especially if it’s a typical cold/viral pattern.
